CLIENT BILL OF RIGHTS

DESIGNED TO RECOGNIZE, PROMOTE, AND PROTECT AN INDIVIDUAL’S RIGHT TO BE TREATED WITH DIGNITY AND RESPECT WITHIN THE COURSE OF YOUR TREATMENT

Before service is initiated an individual has the right to be fully informed verbally and in writing of their rights as a client or patient by the providers of health care or services.

As my client you have the right:

 
 
To be informed of:
  • your rights and responsibilities in advance concerning care and treatment you will receive, including any changes, the frequency of care/service, the nature and purpose of any technique that will be performed, including information about the potential benefits and contraindications
  • Your right to grant explicit consent prior to the start of any treatment or program and be advised of any change in the treatment plan, before the change is made;
  • specific charges for these services, billing policies, payment procedures, and any changes in the information provided at the initial time of purchase;
  • what to do in the event of an emergency; or be referred to another organization if The Body Intuitive is unable to meet your needs or if you are not satisfied with the services you are receiving
To receive:
  • Appropriate and professional continuity of care in accordance with my scope of practice;
  • Respectful, dignifying teatment without regard to race, creed, gender, age, handicap, sexual orientation, veteran status or lifestyle
  • a timely response from the agency to his or her request for service, scheduling, or customer support;
  • A fair opportunity to resolve and voice grievances and suggest changes in service without fear of retaliation or discrimination;
  • A safe environment free from any abusive behavior, neglect or exploitation of any kind by Body Intuitive
To keep confidential:
  • Your records and The Body Intuitive’s policy for accessing and disclosure of records.
  • Your clinical records in accordance with legal requirements and to anticipate the organization will release information only with your authorization or as required by law.
  • Any recordings, photos, or videos taking during client sessions as per request

In addition, as my client, you have the responsibility to:

  • To provide complete and accurate information about illness, hospitalization, medications, and
    other matters pertinent to your health; any changes in address, phone, or insurance/payment
    information; and changes made to health condition, other health/fitness programs entered or
    completed, and any factors relating to the outcomes of treatment
  • To respect the cancellation/rescheduling/no show policies
  • To treat care providers with respect
  • To actively participate in and follow your plan of care
  • To cooperate with staff and ask questions if you do not understand instruction or information
    given to you.
  • To assist the Body Intuitive with billing and/or payment issues and to honor all payment
    contracts
  • To inform the Body Intuitive of any problems or dissatisfaction with services
  • To treat care providers with respect and observe professional behavior at all times
